Consent (Terms of Use), collectively govern your use of our Sites. This Policy
applies to our platforms that link to this Policy. From time to time, we may
change this Policy. If we do, we will post an amended version on this webpage.

Eye exam appointment verification:
If you create an appointment with us, you will provide us
with your name, email address, birth date, physical address, and insurance
verification.
Customer Service:
If you contact our customer service department, you may need
to provide us with additional information so that we can respond to your
particular questions such as glasses, contacts, or referrals.
HIPAA.
Some of the affiliates and brands covered by this
Policy are subject to the Health Insurance Portability and Accountability Act
of 1996 (“HIPAA”). These affiliates and brands may collect additional
information that is considered “protected health information” (“PHI”) as
defined by HIPAA. When the information that we collect is considered PHI, we
will handle that information in accordance with our privacy policy. Please

We use your personal information to schedule your eye exam in a timely and appropriate manner.
This means verifying the correct patient is scheduled, the
correct copay is communicated, and the correct insurance is billed. This
information is used to help us schedule your eye exams and see patients in a
timely manner.
SHARING INFORMATION WITH THIRD PARTIES
We may need to share your personal information that we collect about you in the following ways:
1. The processing of insurance claims for your vision or medical exam.
2. By prescription verification from another entity that is filling your order for glasses, contact lens, or medication.
3. Personal information is not shared to third party unless a signed request or documentation of "records request" is filled out indicating the patient information be disclosed to a specific party.

We retain your Personal Information for as long as necessary to fulfill the purposes for which we collect it, such as to provide you with the service you have requested, and for the purpose of satisfying any legal, accounting, contractual, or reporting requirements that apply to us.

Terms of Use - Communication
What we do?
Cornerstone Family Eyecare provides eye exams for glasses
contacts lens and emergency eye care services. We will communicate with
patients no more than 2 weeks before their exam or as needed per incident. The
communication includes patient scheduling, copay questions, records request, or
office closures. Communication occurs
through phone, email, and SMS/Text.

By registering for an exam, we may call you to confirm your appointment day and time and discuss how this exam will be covered. If you opt-in to SMS messaging, we will use this means as a way to communicate the process of your exam. At any time, you may request to be removed from communication with our office by calling us at 469-405-1779, emailing us Unsubscribe at cornerstoneeyecareoffice@gmail.com, or filling out an unsubscribe form here.
User Representation
By signing up for SMS communication, you will need to
complete our SMS Terms of Use form here. By continuing, you declare that you
are 18 years or older and the person authorizing communication between
Cornerstone Family Eyecare and our potential patient.
PRIVACY POLICY of SMS USE
Before agreeing to SMS communication, please complete the
form to fill to agree (consent) or decline communication or being contact by
SMS or text messages. If you change your mind and you may unsubscribe or
decline future SMS text from Cornerstone Family Eyecare by filling out the
unsubscribe form here.
Text communication is helpful to ensure your exam with
Cornerstone Family Eyecare goes smoothly. We will limit the text messaging to
information pertaining to your patient scheduling, copay related questions, or
exam records request. Messages about your exam will NOT be sent earlier than
two weeks before your exam.
If you need help to opt-in or opt-out of SMS communication,
please call 469-405-1779 for help. This SMS consent information will NOT be
used for the purpose of selling or sharing SMS opt-in phone numbers but
communication only with our office and potential patients only. Text frequency
may vary, and message data rates may apply.
You can opt out at anytime by texting STOP.
Sample messaging
Bad weather: Due to inclement weather and safety of our
patients, we will be closed tomorrow. We apologize but will need to cancel your
upcoming exam tomorrow. Please let us know what day is best for your new exam
OR you may find a open dates to reschedule your exam at
www.cornerstonefamilyeyecare.com. Please text STOP if you'd like to Opt-out of
SMS communication from Cornerstone Family Eyecare.
Appointment reminder: This is a reminder of your upcoming
appointment with Cornerstone Family Eyecare on Thur 02/08 at 2:30 PM. Please
come 15 mins early for your appointment. Text back 1 to confirm your
appointment or 9 if you would like to reschedule or go to

CONNECTED WITH THE USE OF THE WEBSITE OR WITH THE DELAY OR INABILITY TO USE THE
WEBSITE, SERVICES, OR USER-GENERATED-CONTENT, WHETHER BASED IN CONTRACT, TORT,
STRICT LIABILITY, OR OTHERWISE. NOTWITHSTANDING THE FOREGOING, TOTAL LIABILITY

and their directors, employees and agents, against any loss, damage, liability,

without limitation any property, privacy or intellectual property rights. This
indemnity shall survive the duration of your use of the Services.
